APPOINTMENTS:Eye examinations services are provided at Overland Eye Associates by appointment only, during regular office hours. Patients will be scheduled with the doctor of their choice during his available hours of service.
Please call 314-427-1519 to request an appointment. Be prepared to share the name of any vision insurances in which currently enrolled. New patients are encouraged to arrive at least 15 minutes prior to scheduled appointment time to allow for completion of a medical/visual history form. To save time, make a copy of the New Patient Questionnaire form below, fill it out and bring it along. |
